The Guardian Care Management Program is designed to provide patients with comprehensive, individualized support throughout their healthcare journey. Led by Dr. Matt Downen, MD, and Case Manager Courtney Kroeger, the program focuses on proactive coordination, communication, and continuity of care — ensuring that every patient’s medical, emotional, and social needs are met with compassion and precision.
 

Through close collaboration between hospitalists, case managers, specialists, and post-acute providers, the Guardian program helps patients navigate complex medical conditions, transitions between care settings, and ongoing treatment plans. The team develops personalized care strategies that align with each patient’s goals, values, and long-term well-being.
