Lisa Tay, APRN, is a board-certified Adult-Gerontology Primary Care Nurse Practitioner with extensive experience in hospital and post-acute care settings. She earned her Master of Science in Nursing from the University of Cincinnati, specializing in Adult-Gerontology Primary Care, and holds a Bachelor of Science in Nursing from Wichita State University

Lisa currently serves as a Nurse Practitioner with Premier Hospitalists of Kansas, where she assists with the admission and medical management of residents in skilled and long-term care facilities. She is dedicated to delivering comprehensive, patient-centered care, taking detailed histories, conducting thorough physical examinations, and collaborating closely with physicians and interdisciplinary teams to ensure optimal outcomes

With a strong background in cardiovascular care, Lisa has previously worked in cardiology clinics, cardiothoracic surgical units, and cardiac ICU and telemetry settings. Her clinical experience includes caring for patients with complex cardiovascular conditions and multiple co-morbidities, emphasizing evidence-based practice, quality improvement, and patient education

Known for her ability to build rapport with patients and families, Lisa practices medicine using a holistic approach grounded in evidence-based guidelines. She is licensed as an Advanced Practice Registered Nurse in the State of Kansas. She is also a member of the American Association of Nurse Practitioners
